Student Progress Documentation Forms

Consistent and regular monitoring of student progress on daily work provides the classroom teacher with data that can be used to make instruction more effective. The authors of Reading for Success© and Series Launchers© have designed Student Performance Documentation Forms for these highly effective novel studies programs. These forms are complimentary for users of these novel studies. You can download the documentation forms from this website to support the specific novel studies that you are using.

The file for each novel study contains instructions on how to use the form, scoring guide, and the form that can be copied for your use. Space has been provided on the form for you to record a student’s score for redoing the lesson after intervention or remediation. This gives students the opportunity to improve their marks. Many teachers average the two scores so that students can see the results of their efforts to improve.

Data that is gathered on this form provides you with valuable information. If a student’s score is below 80% (mastery), that score should be highlighted. Looking at the scoring pattern for your class or group provides you with information about how your students are performing. If there is highlighting horizontally (across a row); a specific student is experiencing difficulty and should receive intervention for the skills that are causing difficulty. If there is highlighting vertically (up and down a column) for more than one third of the class; the class is experiencing difficulty and should receive additional instruction for the specific skills that are causing difficulty.
